Benefits will be checked prior to your first appointment and coverage will depend on your individual plan. Copayments, coinsurance, and deductible payments are due at the time of service.

Out-of-Network Insurance

To use out of network insurance benefits, please contact your insurance company for verification of coverage. A guide to checking out-of-network insurance benefits will be provided upon request. You will be responsible for billing your insurance company directly for reimbursement. An invoice will be provided to you on a monthly basis with a summary of services for use on insurance claims.
Please note: To use insurance for reimbursement of services, a diagnosis code must be submitted with your insurance claim.
